Job Description

We are seeking a compassionate and dedicated home health nurse to join our team. This position provides nursing care to patients in their homes, supporting them in achieving their health goals while maintaining comfort and independence.

Responsibilities include:

• Conducting patient assessments and developing individualized care plans

• Administering medications and treatments as prescribed

• Providing education and support to patients and families

• Collaborating with physicians and other care team members

• Documenting all patient care according to agency standards

Qualifications

• Current Illinois RN or LPN license

• Valid driver’s license and reliable transportation

• Strong communication and organizational skills
