We're looking for a dependable Physical Therapist (PT) and/or Occupational Therapist (OT) to provide backup coverage for our current therapists when they are unavailable.

This is a great opportunity for a therapist looking to pick up occasional hours while working with a supportive and friendly team!

About the Position

Our Home Health therapists provide personalized care to patients in their homes, helping them improve their mobility, independence, safety, and quality of life.

This position will provide coverage for our regular therapists as needed to help ensure continuity of patient care.

Responsibilities include:

  • Providing physical or occupational therapy evaluations and treatments within the therapist's licensed scope of practice

  • Developing and updating individualized plans of care as appropriate

  • Helping patients improve functional abilities and independence

  • Educating patients and caregivers on exercises, safety, and home care techniques

  • Collaborating with physicians, nurses, and other Home Health team members

  • Completing accurate and timely patient documentation

Qualifications

  • Current Illinois Physical Therapist (PT) or Occupational Therapist (OT) license

  • Graduation from an accredited PT or OT program

  • Valid driver's license and reliable transportation

  • Strong communication and organizational skills

  • Ability to work independently in patients' homes

  • Home Health experience preferred
