About the Position

Our Home Health Physical Therapist Assistants help patients improve their mobility, strength, and independence while receiving care in the comfort of their own homes.

Working under the direction and supervision of a licensed Physical Therapist, the PTA provides treatment to help patients reach their rehabilitation goals.

Responsibilities include:

  • Providing physical therapy treatments according to the established plan of care

  • Assisting patients with therapeutic exercises, mobility, balance, and strengthening activities

  • Monitoring patient progress and communicating changes to the supervising Physical Therapist

  • Educating patients and caregivers on exercises, mobility techniques, and safety

  • Maintaining accurate and timely patient documentation

  • Collaborating with other Home Health team members to provide coordinated patient care

Qualifications

  • Current Illinois Physical Therapist Assistant license

  • Graduation from an accredited Physical Therapist Assistant program

  • Valid driver's license and reliable transportation

  • Strong communication and organizational skills

  • Compassionate, dependable, and comfortable working independently in patients' homes
