age 19 years and older.

If you have a Medicare HMO,
you must go to your doctor to receive your vaccine for FREE!

seasonal flu vaccine for children age 6 months through 18 years.

Price is $10 for flu vaccine through
the
Vaccines for Children Program.
Persons age 6 months through 8 years may require a
second dose
of the vaccine given a month apart.
If this pertains to your child, the nurse will discuss it with you.
A parent or legal guardian must sign the paperwork
for minors.
