Family Case Management Program

Family Case Management is a program for pregnant women, infants, and young children. The goal of the program is to help families in Jersey County stay healthy. FCM can help you and your family find health care, find ways to encourage a healthy life style, help you get the services you need, help you keep immunizations up to date, and provide support for you and your family. Anyone living in Jersey County who is on WIC or pregnant, has low income, or has an infant may be eligible to participate.

FCM also provides home visits for families involved in WIC. A nurse comes to the home once prenatally, once after delivery, and anytime a family transfers into Jersey County. The nurse provides education related to home and baby safety issues as well as monitoring the health of mom and baby.

Another part of the FCM Program is Health Works of Illinois (HWIL). HWIL follows infants through age six and any pregnant teen that is placed in foster care in Jersey County through the Department of Child and Family Services (DCFS). HWIL follows these clients by providing education and support to the foster parents, assisting them in obtaining appointments for preventatvie and acute medical visits with a physician, obtaining past medical records to provide appropriate on-going medical care, and by being a link between the foster parents and DCFS.

HIGH-RISK INFANT FOLLOW-UP assigns case managers to do home visits and follow-up with the families of infants who are discharged from hospital perinatal units - 13,000 infants each year.
